Designed for the first 48 hours of discharge, this package is activated the moment a discharge order is placed and a family reaches out. A dedicated Concierge Specialist goes directly to the facility to meet the client and family, conducts an in-person consultation, and completes a full home safety assessment before the client ever leaves the building. From that moment forward, Heart 2 Heart manages every logistical and administrative detail of the transition home, acting as a full advocate on behalf of the client and family so that neither one has to carry the weight of coordination alone
